Fixed

• Redis connections were not always released after a stream ended (audiohook, file, kvs and media streams), leaking connections over time; connections are now closed deterministically and safely under all disconnection scenarios.
• AudioHook error messages sent by the client were treated as an unexpected server error and forced a disconnect instead of being logged as a warning; the client-originated error message is now handled per spec.

Removed

• external_id field from the POST /api/v1/streams/kvs request body. Requests that still include it are accepted and the field is ignored.
